Learn how to report workplace and work-related incidents and hazards. Some work-related incidents are notifiable under the Work Health and Safety Act 2011 (WHS Act) and must be reported to Comcare immediately.
Template
The Incident Investigation form outlines the process to report and document a workplace incident. It is used to investigate Work health and safety incidents. It should be completed by nominated WHS Site officers in Commonwealth Parliamentary offices.

All workers, including parliamentarians and MOP(S) Act employees, have a duty to take reasonable care that their behaviour does not affect their own health and safety, or that of other persons.
